Independent living can be rewarding yet challenging...

A person faces many issues in deciding to transition from a dependent living environment such as hospitals, board and care facilities, nursing homes, parent’s home, etc.

Our Independent Living Program works one-on-one to provide the information and resources needed to reach your goals.

  • GOAL PLANNING AND PRIORITIZING
    Consumers work with the Independent Living Coordinator to develop an Independent Living Plan (ILP) that outlines goals and objectives the consumer wishes to achieve. The ILP is tailored to the specific needs and abilities of the consumer.

    EXPLORING HOUSING OPTIONS
    What are the consumer’s possible living arrangements and accessible housing needs? We provide information and referral on the various subsidized housing programs in San Diego County. We also provide information on any available funding sources; we have no resources for “emergency housing“.

    COMMUNITY SUPPORTIVE LIVING PROGRAMS
    We explore services or programs that help consumers maintain their independence and safety at home, including, In Home Supportive Services (IHSS), meals on wheels, transportation services, etc.

    CONSUMER ADVOCACY
    We promote self-advocacy and work with consumers who may be experiencing difficulties in obtaining “bureaucratic” community services or programs.

    ADDITIONAL SERVICES
    The Independent Living Program Coordinator (as schedule permits) is available to visit consumers in an institutional setting, including hospitals, nursing homes, board and cares, etc.
